A unique service available at Reuth
Meet Miki Sharon a Social Service Manager and La Grami a Social Worker and Director of the treatment sequence unit at the Reuth Rehabilitation Hospital. For over a decade they have led the unique service of that helps the patients with their discharge from hospital and includes professional guidance and follow-up to ensure that patients continue their rehabilitation process in the community.
The unit's staff assists in training and contacting the parties responsible for the specific community and they support the patients and their families. They help the patients adapt to being out of the hospital environment and prevent deterioration in the patient’s mental or physical condition. The service is provided for free and in five languages!
"The process from the hospital to the community can be problematic and may lead to unnecessarily recurring hospitalizations," explains Miki Sharon. "When a patient comes home there is a state of shock." In hospital patients have constant care and attention, and so the effort and the real challenge is when they go home and sometimes without the right support chaos results. Our service helps in removing blockages, provides emotional support and knowledge of the patient's rights as well as guidance on how to exercise them.”
To date, hundreds of patients have benefitted from this service of close accompaniment after they go home and has been really appreciated by the patients and their families.
